The present study evaluated indication-based alerts during medication ordering. We found an interception rate of 0.25 errors per 1000 alerts. This is difficult to compare directly with other studies since our alerts are a non-random subset of all medication orders. Our interception rate may differ for those errors which may otherwise have been intercepted after submission. It would be very difficult to measure all wrong-patient medication errors as many do not produce harm, so careful measurement of adverse drug events would not suffice.
